What We Do

Wake Connections links you to support and services for pregnancy, first-time parents, babies, toddlers and young children who have not yet started Kindergarten.

Our partner programs can work with your family in your home or in a group setting in the community.

Sometimes, services may be provided over the phone or remotely.

What To Expect

1 Application

First, you’ll complete our application with information about your child and family.

Please answer as many questions as you can. This helps us match your family with the best fit program.

Everything you share is confidential and is only used to help your family connect with services.

2 Referral

After you submit an application, the system will check to see if your family matches the eligibility for a program.

The administrator may contact you if more information is needed.

If a program is a match, your referral will be sent to that program. If there is not a match, we will contact you.

3 Intake

When the program receives your referral, they will contact you to share more about the program and answer any questions.

If the program has a wait list, they will notify you at this time.

If you choose to enroll, the program will explain the next steps to get started with services.
